Costa Rica Health Pass (Pase de Salud)

Costa Rica has introduced a new health declaration form. This was done to help prevent the spread of COVID-19. All travelers traveling to Costa Rica must now complete a Health Pass before entering the country.

Costa Rica is not alone in doing this. Governments around the world have introduced similar medical certifications in response to the coronavirus pandemic. Provided contact and travel information can be used to track foreign visitors more effectively and minimize infection rates.

After registering their details with the online Health Pass, travelers are issued an identification number and barcode, which they must present upon arrival.

Information Required to Enroll in the Health Pass

In order to successfully complete the Health Pass, travelers must provide basic personal information, contact details, and travel information, including:

  • Full name, age and nationality
  • Email address and mobile phone number
  • Arrival and departure date

Travelers are also asked to indicate if they are currently experiencing any symptoms of COVID-19. All information provided must be accurate and true.
